I'm looking for a NYC style mast signal in HO scale like this but with Tri-light heads. Can someone help me out?

You may have to mix and match to create what you want. I think oregon rail supply makes the heads you want, you may want to try NJ international as well.
 
Integrated Signal Systems may also have something like this, too. Don't know about Model Memories. You can find them thru google.
