I found it easier to work 28hrs/wk at Trader Joe's and get full "normal" medical coverage until I hit 65.It's not a "NO X,Y or Z" - We have to have all the hoops jumped through to trigger it. A LOT of plans have moved that way in the past few years. The ACA made insurance more expensive, and the other side's counterpoint lessened some of the restrictions, but costs didn't come down for the consumer. Once a company has increased prices, why decrease? Oh, and now they don't have to cover as much as the ACA.
To be fair, we could go for the Cadillac plan at about 10x the current cost. But that would put a dent in what she's throwing into retirement. She'll do the math on what we spent this year, and see if it's worth double the monthly premium cost to have a lower deductible without much more benefit.

If you are 65+ take full advantage of Medicare and a supplemental plan. Nicki's 12 hours in the ER, with MRI, CT, Xrays, specialists, etc., was fully covered. Her surgery and 5 days in ICU was covered. All the post-op PT, nephrology appts, post surgical follow ups, blood work, etc is all covered.

Yeah, I'd say about 50% of all meat dishes use that combo. Usually equal parts of shoyu, mirin, sake and a little sugar if needed. Since we use that a lot, there was enough for me to make the dish. I keep forgetting to check to make sure, but we had unopened bottles in the storage. I even screwed up and used the wrong side of the measuring cup. I used the gradients used for sugar instead of liquid, so I had added more than two times the amount of sake to the mirin, so had to dump it all and start again.soy sauce, mirin or sake or both, and a little sugar.
