Good Morning Al. Clear and 62°, supposed to be up to 96° later today. A bit early for that kind of temperature, but it is forecast to be one day only. Today is Palm Sunday for many.

Most of yesterday's plans went off without a hitch. I mowed the entire east side and part of the northeast around the garden. I trimmed up some more firewood from crooked and misshaped branches; all of the current pile is taken care of now. Won't have time today for either as there is housework on the agenda. Dishwasher and laundry tops the list. OK though, as I am not ready for the hot weather yet. I might have time to pump the rainwater off the top of the pool cover since opening day is coming up in about three weeks.

I had a terrible experience yesterday with the chair rental place that I have used several times in the past. They have undergone another ownership change with new national corporate owners. For over 30 years, they were a locally owned company called "Ducky Bob's". Then they were sold to a regional corporation and things were basically still the same. Now they are owned by a much bigger company and the weekend phone is answered by an answering service in what I surmise is Korea based on the accent of the girl on the phone. While she could ask questions from her rehearsed script, there was no deviation. I asked questions which were ignored while she went to her next one or repeated the one that she just asked because I responded with a question instead of giving her an answer that she understood. Very frustrating. A bit later I saw a missed call on my phone, which I don't carry, from a somewhat local area code. I called and spoke to a gentleman who worked locally. But he couldn't help because he was just the local dispatcher and didn't have access to the sales or anything. He just saw the message sent to them and called back to see if there was anything that he could help with. He was helpful as much as he could be, but I will have to wait until Monday for a proper callback. Both of the previous owners had proper staff available on Saturdays.

Out in the train shed yesterday, I was dismayed to find that the beer fridge had bit the dust. Bummer. I moved a couple of days supply into the regular in-house fridge. I ordered a new one online and it will be delivered Wednesday. I think that I actually broke it when chipping ice out instead of running a regular defrost cycle on Friday. Otherwise I just ran trains. The camera just isn't getting used right now.

Marlin - My next door neighbor who is now a retired policeman, used to practice shooting on his property with no issues. He stopped because he started hitting spent bullets with his mower. Soon afterward his PD built their own range around 1989 or so.
Joe - I've had the best service from a locally owned independent pharmacy. I have tried Walgreen's, Target, Walmart and Kroger pharmacies in the past with mixed results. Walmart was the worst, as it was way too busy with what appeared to be a rather bizarre and questionable clientele.
Regarding the NP's and PA's; my primary care physician uses an NP who is really good. She gets things rolling with us and then he joins in as well. I always see both of them at the same time and they have a back and forth banter between them and with us that I find really helpful. He has a second one who I don't see as often, but it's always been the three of them together. They all work well as a team.

Everyone have a great Sunday

should be, focus on prevention and not just treating the symptoms.
Interesting that you bring this up. Medicare and most Private and Employer Insurance policies do not pay for Preventive Care...If a test is coded as a "screening", it's denied. That includes certain blood tests more than once every so many years.

The other side of the coin is that in some instances preventive care doesn't stop stuff from happening, Arthritis, for instance, and heart disease.

All the folks at the VA Clinic know what they are doing and get it done mostly 'right now'.
VA is hit or miss. Depending on where you live and what clinic you go to. For instance, My son uses the private doctor affiliate option, because the VA clinic in Jacksonville NC is inadequate for the volume of users...The alternative is to drive to Fayetteville, NC., (Also high volume). The service is good, the waiting time, not so good. (He works out of Havelock, NC.) Other son, still active duty, uses Tri Care.

Morning again,

Mom is at mass at the facility. Non-profit catholic care center. Nice for what it is. Waiting in lobby for when she is out.

Thus far my few experiences with Medicare for the wife actually suck. With my insurance, we only have to have part A coverage (which my plan covers better). Had to buy a part c as my plan prescription coverage doesn't meet something with Medicare. Part D is cheap enough and 4 of her 6 drugs are no cost. One is $79 for 90 days, $10 on mine. Her last one is $600 for 90 days until we hit $2000. Free on my insurance after the city worked a deal with a local pharmacy. Don't understand how $2000 is saving us any money. Also can't use drug company coupons if you have Medicare. I am not looking forward to when I'm eligible next year.

My blood work does show what is going on. Myeloperoxidase, Lp-PLA2 Activity, Lipoprotein Fractionation (lipid size and particle count), Vitamin D, Coenzyme Q10, etc.

There is also a blood base cancer screening that can detect over 50 cancers. Catching pancreatic, lung or stomach cancer before stage 2 is almost always successful. The test should be covered by Medicare in the next year. It just became covered by Tri-Care.

That's a Walmart trademark. What was that slogan..."By our clients we shall be known"?

Most NPs and PAs I have encountered are knowalegable and competent. The problem is the Medical Schools, and their admission process.
My cousin wanted to be a Doctor, back in the '70s. He had good grades and scores, took the appropriate Pre-Med for the time, and literally applied and interviewed at 16 Medical schools, before University Chicago accepted him in their DO program. Back then DOs were looked down on by MDs. (Now they are in the same practices).

My primary care Doctor is a DO. She is the best primary care doctor I have ever had. In the few years I have been with her she has spent more time with me than any doctor has in my entire life. I even have her personal cell phone number so that I can report my daily blood pressure to her. I send her text messages, but I have never called her, but she has called me several times.

Your system is unique because of your work benefits. Some of what I comment on won't apply for that reason.

I have been on Medicare since 2013. I transitioned from a limited managed car retiree policy from Amtrak, that was terrible. (lowest bid).
Being Cheap, I took Traditional Medicare, with a supplement and drug plan with the Teamsters. While the premiums were low, there were substantial deductibles and co-pays. After a year, I signed up with the Railroad Union plan as a supplement, and kept the Teamster drug plan until it shut down. (I looked at the Part C Plans, but most had "Cost Sharing", for the same premiums as I had, and restrictions on providers.)
My coverage, covers 100%, of Medicare, no deductibles, and Part D is through a Blue Cross Affiliate, with no deductibles, and co pays on Tier 3-4-5 Drugs. (Standard).

The $2000 limit on out of pocket costs, excluding premiums, only helps when you use one of the expensive drugs you see advertised on TV. My wife uses a Drug that retails for $6000, for 30 days. Her cost sharing is fulfilled in January...used to have to wait for March under the old rules. If you are fortunate not to have something that requires these drugs....then it doesn't mean anything..... Medicare, is a one size fits all program modified to allow private sector, for profit insurers to offer managed care policies, that might limit care, for the same overall cost. (But appeal to those who prefer private enterprise).

Terry has had quite the medical year. Strong as an ox from the day I met her in 1973 until this latest bout of Trigeminal Neuralgia and the new pain management drugs available. In February when the medical experts thought she should go to assisted rehab for pain management for 6 weeks we decided no. I have heard so many horror stories about the recommended facility that when we confronted them with the no and articles pulled from online the response was, that’s all hearsay! That’s when we switched her medical team.

All the years I have either been a patient or selling services to the medical community it’s the 1st time it entered my mind that these folks were getting kick backs. Medicare had them highly rated in the area we live and I shake my head. The facility in our area is brand new in 2024 and we both feel they were driving residents to it!

But with all the set backs, recoveries and crap she has gone through maybe she should have gone somewhere but after this neither of us trust our medical teams. I even have concerns about my Endocrinologist who has become a close friend of Terry’s.

My Neurologist is a DO. He is also a professor at the Rutgers School of Medicine and Dentistry, in New Brunswick. He teaches MD candidates....Guy is good, knows his stuff.

My experiences with "Extended Care and Rehab Facilities has not been favorable.
MIL had Hip Surgery. Social Worker employed by Hospital recommended an extended care center for inpatient rehab. Hospital Corporation has one right on their campus adjacent to the Hospital. They recommended that she be admitted to another facility, that was a recently privatized County Facility. (They did not think the MIL's Part C plan with Aetna paid enough). She was admitted to the other facility, and assigned a bed. This was on a Friday. On Monday we inquired why her rehab had not begun? Their answer was she's not sufficiently recovered. Thursday, they began with a short session, No rehab Friday. Friday afternoon after hours, we were notified that Aetna had issued a "Cut Letter", ending coverage in 48 hours (Sunday). If we wanted rehab she had to go on self pay. Insurer sent us their termination notice. Reason stated: MIL was not responding to Rehab. On Saturday, after a threat of litigation, we had her back into her Assisted Living Facility. On Tuesday, she was in outpatient Rehab not connected with the Facility. She responded and was walking with a cane after 10 sessions.

STAY AWAY FROM EXTENDED CARE AND REHAB FACILITIES....AVOID ASSISTED LIVING FACILITIES. They exist to bankrupt their clients (Patients) and families.

The Masters Tournament on TV. The Orioles host the Blue Jays on the PC. A traditional Sunday dinner, roast chicken in the oven.

Life is good!

LIke I said earlier, care facilities are warehouses for the sick and elderly. Some go there to spend their waning years to die and some survive their rehab. The "business" model exists to collect Medicare and Medicaid.

I honestly believe multi-generational households are healthier. Hospice at home is more dignified.

These are just my opinions.
