QSI lights not working properly

I just completed my track, so putting lots of locomotives into service for first time. While I like all of the features of the QSI, the Soundtrax equipped locos are easier to program and more reliable for me so far.

Anyway, I have put four QSI equipped Atlas locomotives into service without too many difficulties, although all the different versions create a lot of confusion. I just put a new Atlas SD-35 into service and having two issues. Using NCE Powerhouse Pro 5 amp system with PowerPax programming track booster, and JMRI Decoder Pro software. I tested the loco with only that loco on the track, programming track, and mainline.

For this loco only, I can not read CV's on the programing track. When trying to use the programming track I get a message, "No decoder found". I was able to program the 4 digit address on the main. However, lights, both front and rear, go off and on unexpectedly. This is not a flicker. Sound is steady, speed is steady. Lights come on and stay on for a few minutes, then go off and stay off for several minutes. Any ideas?
 
I would first try to reset the decoder back to defaults. Sometimes decoders lose their minds, and the only way to recover, is set back to factory defaults.
